We welcome you to Dr. Lindeman’s family and cosmetic dental practice. On your first visit, we will conduct an initial exam

Complete the patient registration page.

If you were referred, please bring your referral paperwork and any X-rays if applicable.

List of medications you are presently taking including heart medications, aspirin or blood thinners.

I

Notify the office in advance if you have a medical condition which may require you to be pre-medicated prior to dental appointments.

I

If you have dental insurance, bring your dental insurance card or information. This will save time and allow us to process your dental claims.
